I suffered with RSD for 4 years due to an injury with my right thumb. I had 8 surgeries and 12 stellate ganglion nerve blocks. Therapy has it's place, but you have to be very careful about the therapist and type of therapy program. Too agressive is not good. Also look for underlying problems it may have created. As in my case from all the swelling I developed Carpel Tunnel which caused the problem to be even worse. And because of it not being diagnosed properly my thumb was eventually amputated because I could not take the pain any further. Even more coincedental is now my brother is also suffering from it. It has now been more than a yr. for him and he lives on Demoral. Get a good pain mgmt. specialist who only deals with that. I live in a big city with great specialists.
